The game starts with Sebastian, Joseph, and Kidman traveling with police officer Oscar Connelly to Beacon Mental Hospital to probe a serial killing. After they arrive, the trio enters and discovers mutilated bodies scattered throughout the hospital except for one survivor, Dr. Marcelo Jimenez. Before passing out, he mutters the name "Ruvik." Sebastian reviewed the security footage and discovered that three police officers were murdered in an instant by an unidentified man. After the attack, the man spotted the CCTV camera and vanished. Suddenly, Sebastian sensed someone behind him. He quickly turned around and came face-to-face with the same mysterious man who then assaulted him. Sebastian wakes up hanging upside down and sees Sadist, a big man who tells him to play dead. After Sadist leaves, Sebastian uses a knife to untie his legs and grab the keys.
